Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

HeartPath Home Care is required by law to protect the privacy of your protected health information ("PHI"), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your health information

TreatmentSharing your child's plan of care with the nurse or CNA assigned to your home, or with your child's physician.
PaymentSubmitting claims and supporting documentation to Georgia Medicaid or another payer.
Health care operationsNursing supervision, quality review, staff training, and case coordination.
Business associatesVendors who perform services for us under a written agreement requiring them to protect your information.
Required by lawPublic health reporting, suspected abuse or neglect reporting, health oversight, court orders, and similar legal obligations.
To avert a serious threatDisclosure necessary to prevent a serious and imminent threat to health or safety.

Uses that require your written authorization

Most uses and disclosures of psychotherapy notes, uses for marketing, and any sale of PHI require your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted on it.

Our duties

We are required to maintain the privacy of PHI, to notify affected individuals following a breach of unsecured PHI, and to abide by the terms of the notice currently in effect. We reserve the right to change this notice and to make the revised notice effective for information we already hold. Revised notices will be posted on this page.

How to file a complaint

If you believe your privacy rights have been violated, contact us using the information below. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, 200 Independence Avenue SW, Washington, DC 20201, by phone at 1-800-368-1019 (TDD 1-800-537-7697), or at hhs.gov/ocr/complaints.

We will not retaliate against you for filing a complaint.
