Privacy Policy

This policy explains what HeartPath Home Care ("HeartPath", "we", "us") does with information collected through heartpathcare.com. Health information we hold as a health care provider is governed separately by our Notice of Privacy Practices.
Information we collect on this website
This website hosts five forms: the eligibility check at /start, the referral form at /referrals, the nurse application at /careers, the Path Finder at /path-finder and the Caregiver Pay Check at /caregiver-pay-check. We do not sell information, and we do not share it with advertisers.
- Information you volunteer. If you call 678-459-2590 or email us, we receive whatever you choose to tell us.
- Standard server records. Our host records routine technical data such as IP address, browser type and pages requested, used to keep the site running and secure.
- What our forms collect. Your name, phone number, email address and, depending on the form, your county, your role, or your relationship to the child. The careers form also accepts a resume you choose to attach.
- What our forms deliberately do not collect. We do not ask for a child’s name, date of birth or diagnosis anywhere on this website. Those details are taken on the phone, where they belong. One form goes further than the others and you should know exactly how: when you submit the Caregiver Pay Check, it sends us your child’s age range and one general care category you picked, such as “feeding tube, tracheostomy, ventilator or oxygen.” That is a category, not a diagnosis, and there is no child’s name attached to it. We ask because approval for the paid caregiver option depends on your child's age and level of care, so without it we would be calling families back to tell them something we could have told them on the spot.
- Answers that never leave your browser. The eligibility self-check on our home page, the care-need questions in the Path Finder, and the household income figures entered into the Path Finder are all calculated on your own device and never transmitted to us. If you ask us to call you at the end, we receive your contact details and the recommended pathway, not your income. The Caregiver Pay Check works differently, and we would rather say so than let you assume otherwise: its answers are summarized and sent with your application, as described above.
Analytics runs only after you accept cookies. Nothing loads before you choose, and you can decline. Session-recording tools are blocked outright on the Path Finder and the Caregiver Pay Check, because those are the pages where families enter income and health answers.
Links to other sites
Our eligibility check, referral form, careers page and both family tools are hosted on this website and are covered by this policy. Some pages link out to Georgia Medicaid, the Social Security Administration, Babies Can’t Wait and other organizations. Those sites have their own privacy notices and this policy does not cover them.
How we use information
To respond to you, to determine whether your child may qualify for the Georgia Pediatric Program, to provide and coordinate care, to meet legal and Medicaid program obligations, and to operate this website.
How we share information
We share information only as needed to provide care and run the business, for example with Georgia Medicaid and its contractors, your child's physicians, and service providers who work on our behalf under confidentiality obligations. We also disclose information when the law requires it.
Children
This site is written for parents and caregivers. We do not knowingly collect information directly from children through this website.
Your choices
You can ask what we hold about you, ask us to correct it, or ask us to stop contacting you, by phone or email. Rights specific to protected health information are described in our Notice of Privacy Practices.
Security
This site is served over HTTPS. No method of transmission or storage is perfectly secure, so please do not send your child’s medical details by email or through any web form. Beyond the single care category in the Caregiver Pay Check described above, our forms do not ask for them, and you should not add them. Call us and we will take those details securely.
Changes
If this policy changes we will post the revised version here with a new effective date.
