Good to know

  • GAPP runs from birth to 21, so a child can be on Babies Can't Wait and GAPP at the same time.
  • Babies Can’t Wait ends at three. GAPP does not. Referring around age two and a half gives us time to begin the GAPP process before BCW ends, which reduces the risk of a gap in support.
  • Families told they earn too much for Medicaid can still reach GAPP through Katie Beckett, which looks only at the child's needs. The family files that application themselves. HeartPath does not file it. We point them to it, and once they are approved we take the GAPP paperwork from there. Georgia lists Katie Beckett for children 18 and under, so that pathway closes before GAPP's age-21 limit.
  • Not sure a patient qualifies? Refer anyway. We screen for potential GAPP eligibility and verify Medicaid status, then tell you what is possible, at no cost to the family.
  • We aim to respond to referrals quickly, during business hours, Monday through Friday.

Georgia Medicaid decides who is approved, what services are authorized and how many hours. HeartPath cannot approve or deny anyone. What we can do is tell you and the family honestly where things stand.

What we do

Skilled nursing at home, and the paperwork off your plate.

In-home skilled nursing

RNs and LPNs deliver private-duty nursing in the family's home for children under 21 with complex medical needs: trach and vent, feeding tube, oxygen, seizure management and more.

We own the paperwork

GAPP prior authorization, the Plan of Treatment, medical necessity and Medicaid coordination. We coordinate the GAPP process from referral through authorization and staffing, so your team and the family are not buried in forms.

We coordinate with your team

We work alongside discharge planners, case managers, social workers and physicians, and keep you updated on where each referred patient stands.

Continuity as they grow

We support medically complex GAPP children over time, including the transition toward COMP and NOW waivers as they age out of the pediatric program.

We verify and file

We confirm Medicaid, or point the family to Katie Beckett if that is their door, then coordinate and prepare the GAPP prior authorization and Plan of Treatment.

Care starts, you stay informed

We staff the case, begin nursing in the home, and keep your team updated on the outcome.

What we staff

Skilled hours and personal care hours

GAPP covers two different kinds of hours. Knowing which a case needs helps us place the right person the first time.

1Skilled hours

RN or LPN

Tracheostomy and ventilator care, feeding tube and central line care, seizure protocols and neurological monitoring, medication administration and clinical oversight.

2Personal care support hours

CNA

Bathing, dressing, grooming and feeding, mobility, transfers, turning and repositioning, everyday activities of daily living, supervised by our nursing team.

3Personal care support hours

Paid family caregiver

Where the family prefers, a parent or family member can be hired and paid for those personal care hours instead, which is less commonly approved for very young children.
